It began with a single denial that never should have happened.
Someone in my family went in for care and paid their copay. Weeks later, a second bill showed up for the same visit. The claim had been denied over a billing mistake, and instead of fighting it, the practice just billed the patient. We appealed, and it got overturned. We'd been right all along. And it kept happening, including a drawn-out prior-authorization fight.
It was the same story every time. Denials like it kept happening, and the practice had no practical way to work them, so it billed the patient instead. Everyone walks away frustrated, and half the time the practice still doesn't get paid.
And this isn't random. Claim adjudication is increasingly automated, and denials are rising at scale — the volume now outpaces what most practices can work by hand. (ProPublica, 2023; U.S. Senate Permanent Subcommittee on Investigations, 2024)
Now think about the doctor. They've already done the work and covered the cost, and a big share of what they're owed may never get paid. More than half of practices now have over 10% of their claims denied, and it keeps climbing as adjudication automates. (HFMA, 2024; Experian, 2025) Every community leans on its local doctors, and almost none of them have the tools to keep pace.
